7 Aims of Antenatal care
1) Detect and manage pre-existing maternal disorders
2) Prevent, detect, manage maternal complications of pregnancy
3) Prevent, detect, manage foetal complications of pregnancy
4) Assess maternal and foetal wellbeing
5) Provide advice and education on the normal symptoms of pregnancy
6) Provide lifestyle and health promotion advice.
7) Prepare women for labour and parenting
What advice is given at the booking appointment?
Folic acid supplements, food hygiene, lifestyle advice
When is the booking appointment?
10 weeks
What examinations are done at the booking appointment?
Height, weight, BMI, BP, Urine
What investigations are done at the booking appointment?
Screen for infections (Hep B, HIV, rubella,syphilis)
Screen for anaemia and haemoglobinopathies
Blood group, rhesus D status, atypic red cell alloantibodies
Downs Syndrome screening
Ultrasound- confirm viability, gestation, no. of foetuses
When does the 2nd trimester begin?
16 weeks
What scan is done at 20 weeks?
Anomaly scan
When does the 3rd trimester begin?
Week 28
What investigations are done at the 28 week appointment?
BP + Urine (pre-eclampsia detection)
Foetal heart auscultation and foetal movement
Anaemia and atypical red-cell antibodies
Offer iron or anti D if necessary
Measure and plot symphysis-fundal height
What investigations are done at 31,34,36,38 week appointments?
BP+Urine
Measure and plot fundal height
Foetal heart auscultation, foetal movement
Info on labour, birth plan, breast feeding
Palpation of abdomen to find baby’s position
What investigations are done at 40,41 weeks?
Info on prolonged pregnancy and induction of labour
Offer membrane sweep to bring on labour
BP+ Urine
Measure and plot fundal height
Foetal heart auscultation, foetal movement
What investigations/scans are done 10-13 weeks for Downs Syndrome
Blood test for BhCG and PAPP-A
Nuchal thickness on ultrasound
Maternal details (age, weight, gestation, family origin, smoking)
What investigations/scans are done 14-20 weeks for Downs Syndrome
Blood test for hCG, UE3, AFP, Inhibin B

When can chorionic villus sampling be done?
11-13 weeks
When can amniocentesis be done?
> 15 weeks
