Abreva
Docosanol
Antiviral - shortening the time to heal (HSV 1)
Raised fluid filled lesions, forming a light or yellow crust over the lesions (2 weeks)
Cold sore or fever blisters on face or lips
Apply to affected area or face or lips at the first sign of cold sore/fever blister. Use 5 times a day until healed.
MAXIMUM: 10 days

SE: GI (take with food, milk, antacid),GI ulceration and bleeding (age >60, prior GI bleed, concurrent use of anticoagulant or ASA, higher dose or longer duration, moderate use of alcohol) - bleeding in stool
CI: MI, HTN, stroke, HF (fluuid retention/edema), kidney
MAX: 10 (pain), 3 days (fever)

Congestion - cold or allergy
Alpha 1 adrenergic receptors - vasoconstrictions (reduce blood flow to nasal)
prolonged use - rhinitis medicamentosa (rebound effect )
CI: MAOI

Ask pt: concurrent blood thinner, asthma or ulcer disease, intense or prolonged pain, d/c if rash or difficulty breathing

Diphenhydramine Allergy 1-2ndoses q4-6hr Max: 300mg DPH/day \+phenylephrine - temp. treatment of sinus congestion
SE: drowsiness, dry mouth
Decongestant: + tachycardia and insomnia
NMT 7 days or in presence of high fever
May cause excitability in children (CI: <6yo)
Decongestant - X pts with HTN, DM, HF
Benadryl topical
Diphenhydramine + zinc acetate or camphor
Temp. relief of itching and pain associated sith insect bites, rashes, poison ivy, poison oaks, poison sumac, allergy, minor skin irritations
Apply to affected area 3-4 times a day
X children <6yo
X eye, mouth, nose, broken, blistered, or oozing area of skin
SE: local irritation

SE: n/v, epigastric distress, aggravation of gout and delayed coagulation

Capsaicin
Topical analgesic - RA, OA, shingles, diabetic neuropathy
Apply to affected area 3 to 4 times a day.
Wash hands following application or use disposable latax glove. Transient burning may occur upon application but should disappear in several days.
May take up to 2 weeks for an effect.

Menthol, camphor, benzocaine
Temporary relief of cold sores and chapped lips: apply to affected areas as needed. Do not use in or around the eyes. Do not share containers to avoid spreading the herpes simple 1 virus. NMT 14 days
