what needs to be on a signed order?
Legislation does not specify the details that need to be included on a signed order, however…..
-Name, quantity and where it is not apparent, formulation and strength
-Name and address, trade business or profession of the person to whom medicine is supplied.
-Purpose for which the medicine is sold/supplied
- Signature of requisitioner/purchaser
-Date of sale/supply
record keeping of signed orders?
Pharmacists required to keep either the order/invoice (or a copy) relating to the supply of POMs; or make an entry in the POM register
Records and/or signed orders need to be kept for two years following the date of supply
Good practice to do both for audit purposes
what does the entry for POM register need to include (signed order- record keeping)
An entry in the POM register must include
-Date the POM was supplied
-Name, quantity and where it is not apparent, formulation and strength of the POM supplied
-Name and address, trade, business or profession of the person to whom the medicine was supplied
-The purpose for which it was sold or supplied
Where the supply involves a Sch 2 CD a separate entry needs to be made in the CD register (see later)
For audit purposes, should include how much you charged
Signed patient order - who does it apply to and how does it work?
Applies to optometrists and podiatrists
Pharmacists can supply certain POMs directly to patients in accordance with a signed patient order from any registered optometrist or podiatrist
The medicines must be one which can be legally sold or supplied to these professionals
The signed patient order is not a prescription so the usual prescription requirements are not needed
As the medicine is going to a patient, then it needs to be labelled accordingly as a dispensed medicinal product

Following the Shipman Inquiry, standardised Controlled Drug requisition forms were introduced
In England, it is unlawful for a supply to be made, in the community, against a non-standard requisition form (applies to Sch 2 + 3 CDs for both human and veterinary use)
The requirement to use the mandatory form also includes paramedics and the person in charge or acting person in charge of an organisation providing ambulance services
Hospices and prisons are exempt from the requisition requirements and are not required to use the mandatory form
what are standard controlled drug requisation form?
FP10CDF - England
CDRF/GP10A – Scotland; private or NHS supplies, respectively (available from Local NHS Health Board)
WP10CDF – Wales (available from Local NHS Health Board)
Professionals can complete the FP10CDF online or download onto local drives prior to the form being completed, printed and signed in wet ink
what are the legal requirements for controlled drugs requisition?
The LEGAL requirements:
Signature of the recipient
Name of the recipient
Address of the recipient
Profession or occupation of the recipient
Total quantity of drug (doesn’t have to be both in words or figures)
Purpose of the requisition
Processing Requisition Forms (markingand sending to the relevant NHS agency)
Legal requirements
A LEGAL requirement to:
Mark the requisition indelibly with the supplier’s name and address (i.e. the name of the pharmacy) when received – where a pharmacy stamp is used, this must be clear and legible
Send the original requisition to the relevant NHS agency using the FP34PCD form
Good practice - retain a copy of the requisition for 2 years from the date of supply
Community pharmacies require a private controlled drug account number when submitting FP10PCD private prescription forms and FP10CDF requisition forms. Different to the NHS account number used to submit NHS prescriptions
What are controlled drug requisition requirements:
Faxed or photocopied requisition NOT acceptable
Legislation requires that a requisition in writing must be obtained by the supplier BEFORE delivery of any Sch 2 or 3 CDs to most recipients (except other registered pharmacies but good practice to do so – i.e obtain an approved requisition form)
In an emergency, a Dr or Dentist can be supplied Sch 2 or 3 CDs on the undertaking that a requisition will be supplied within the next 24 hours
Messengers must have written authorisation that empowers them to receive the drugs on behalf of the recipient
Hospital pharmacy requisitions from a ward or department that are presented to a pharmacy that is a separate legal entity must use requisition form FP10CDF
The Home Office has advised that the person in charge or acting in charge of a hospital can issue a yearly ‘bulk’ or ‘global’ requisition form to the separate legal entity that supplies its wards or departments. The wards or departments then draw on this throughout the year using CD requisition books with duplicate pages
Where the person in charge, or acting in charge of a hospital issues and signs a requisition, this must also be signed by a doctor or dentist employed or engaged in that hospital

when does processing requirements not apply ?
Processing requirements do not apply when the supply is made:
By a person responsible for the dispensing and supply of medicines at a hospital, care home, hospice, prison or organisation providing ambulance services who must mark and retain the original requisition for 2 years
By pharmaceutical manufacturers or wholesalers
Against a midwife supply order
Against veterinary requisitions (the original requisition should be retained for 5 years)

midwife supply order : what CD can they request for and what does the order need to contain?
A registered midwife may request the following CD’s:
Diamorphine
Morphine
Pethidine
Order must contain the following:
Name of the midwife
Occupation of the midwife
Name of the person to whom the CD is to be administered or supplied
Purpose for which the CD is required
Total quantity of the drug to be obtained
Signature of an appropriate medical officer
what is naloxone?
Naloxone is an opioid antagonist and is licensed for the treatment of suspected acute opioid overdose
Staff engaged or employed in lawful drug treatment services can obtain naloxone from a wholesaler and make direct supplies to patients without a prescription, PGD or patient specific direction
Lawful drug treatment services include needle and syringe programmes provided by pharmacies, and those pharmacies providing drug treatment services (e.g methadone)
