Non-fermenting Gram negative rods (4) Flashcards
Name the non-glucose fermenting GNRs (4)
1) Pseudomonas aureginosa
2) Burkholderia cepacia and B. pseudomallei
3) Acinetobacter baumanni complex
4) Stenotrophomonas maltophilia
What makes Acinetobacter baumanni complex culture features different from other NF GNs?
- NLF GNR
- Strict aerobe
- Chunky Coccibacilli
What diseases does the Acenitobacter baumanni complex cause?
Nosocomial infections
- VAP
- UTI in catheters
- Wounds-Associated with contaminated soil.
Some soldiers that fought in the Iraq war had it.
What are the culture characteristics of P. aureginosa?
- NLF GNR
- Grows metallic colonies on blood agar (pyocyanin)
- Grows on MAC but does not ferment
- Grows very well on humid surfaces
Does P. aureginosa have intrinsic resistance?
Yes!
Common P. aureginosa infections?
- VAP (#1 agent)
- Chronic lung infections (specially in CF)
- Endotracheal tube infection
- Skin infections (wounds/burns)
- Eye infections (kerititis- inflammation of the cornea)
- Otitis media- middle ear infection
What are the culture characteristics of Stenotrophomonas maltophilia?
- NLF, GNR, non-glucose fermenter
- maltose oxidizer
What are the diseases caused by Stenotrophomonas maltophilia?
Nosocomial pathogen in immunocompromied/debilitated patients
- UTI (catheters)
- Colonizes endoscopes (tube inserted through natural body openings)
- Bacteremia
- Pneumonia
- Wounds
What are the culture characteristics of the Burkholderia cepacia complex?
- NLF, GNR, non-glucose fermenter
- Grow well on the BCC selective agar because they are resistant to polymyxin B and gentamicin (aminoglycodise)
Does Burkholderia cepacia complex have intrinsic resistance?
Yes.
Beta lactamase and aminoglycosides!
Diseases associated with the Burkholderia cepacia complex?
- Deadly to CF patients!!
- VAP
- Infects Nebulizers and Hospital cleaning products
- Pneumonia
- UTI
- Bacteremia
- Septic arthritis
- Peritonitis (infection in peritoneum/abdomen)
Burkholderia pseudomallei
- NLF, GNR
- resistant to polymyxin B
- BSL 3 agent!!
What disease does Burkholderia pseudomallei cause?
Melioidosis
*Spreads by contact with infected soil or water
*More common in Southeast Asia, Northern Australia
*Protean manifestation: Wide range of infections: cutaneous, pulmonary, bacteremia or asymptomatic
What are the risk factors of Melioidosis?
immune suppression in general
diabetes
kidney, liver, blood disorders
thalassemia (genetic disorder that reduces hemoglobin carrying RBCs)
