How do NOACs work?
A
Inhibit factor 10a so fibrinogen clot cannot be formed

What are the common uses for NOACs?
A
- VTE prophylaxis in hip and knee sugery
- VTE treatment
- VTE recurrence prophylaxis
- Prevention of stoke and systemic embolism in those with non valvular AF

What are the contraindications of using NOACs?
A
- Active bleeding
- Hepatic impairment (coagulopathies)
- Renal impairment
- Valvular heart disease
- Unstable blood pressure
- Active bleeding/stroke in those with ACS
- Pregnancy

What are some interactions of NOACs?
A
Aspirin Apixaban Diclofenac Warfarin Clopidogrel St Johns Wart (CYP3A4 inducer therefore will reduce levels of Rivaroxaban)

What are the side effects of NOACs?
A
GI Upset Dizziness Headache Haemorrhage Renal impairment Pain in extremities Rash Pruitis

How are NOACs excreted?
A
Metabolised by liver
Excreted in faeces and urine

Why are NOACs used instead of warfarin?
A
No need to measure INR
However no antidote available in bleed
