Name a post synaptic NMJ disorder.
Myesthenia gravis: CMAP normal;
Decrement with 3Hz RNS, EMG: normal or early recruit
MUAPs- unstable or increased jitter or BSAPP
Name pre synaptic NMJ disorders
Lambert Eaton and Botulism: CMAP decreased
Decrement 3Hz RNS, Increment 50Hz RNS (huge with LE)
EMG: LE normal or early; Botulism- fibs and psws
MUAPs- unstable/increased jitter or BSAPP
What causes Myesthenia Gravis?
post synaptic- not enough ACH receptors to bind to
test proximal mms RNS ‘upper trap/spinal accessory nerve
What causes Lambert Eaton syndrome?
pre synaptic- not enough ACH released- attacks voltage gated Ca channels
What does fast RNS cause?
increased Ca influx which results in increased ACH getting released
