upper limb ataxia with brisk reflexes and a spastic paraparesis with bilateral upgoing plantars.
emergency MRI of cervical spine
A 60 year old man was kicked in the head a few weeks ago, after which he was concussed for a few minutes. He smells of alcohol, he is drowsy and brought to casualty by his wife who is concerned about him.
His eye opens to command, has incoherent verbalisation and mild grade 4+ right sided weakness with unequal pupils.
