T(15,17)
Matched sibling transplant
Bad prognosis of AML
Wbc >100.000
Secondary
Monosomy 7
CML
Philedelphia chromosome,
BCR-ABL fusion protein: P210
Ph+ALL: P190
Phases:
1. Blast: >%30, look like all or aml, extramedullary disease symptoms
2.Accelerated: progressive, splenomegaly
Blasts:10-30, baso+eo>%20
3.chronic: peripheral/ marrow blasts <%10
Thrombocytosis, leukocytosis
Treatment: Gleevac (imatinib) :tyrosine kinase inhibitor that blocks BCR-ABL
Transplant is the choice for peds
Additional chemo if progresses: IFN, Ara-c, hydroxyurea
JMML: juvenile myelomonocytic leukemia
Young kids
Assoc with NF1
Lab:High HgbF,hypersensitivity to GM- CSF, monosomy 7, no BCR-ALB, blasts+pros <%20, monocytosis
Like a stem cell leukemia, but acts like: MDS more than leukemia !
Treated with SCT, but very often fatal
