With cast care what condition are we worried about?
Compartment syndrome
What do you assess for compartment syndrome?
- 5 P’s
- Pain, Pallor, Pulseless, Paresthesia, Pressure (increased)
How do we treat compartment syndrome?
- Release pressure in cast to fix
What is cerebral palsy caused by?
improper development or anoxic insult to brain in inutero, during delivery, or months after
Is cerebral palsy permanant?
Yes
Who are most likely to get cerebral palsy and why?
Prematures often times get this because intraventricular hemorrhage (brain bleed)
Tx for cerebral palsy
- Therapy - Occupational, physical, speech; Case manager - support services;
- early intervention and assistive technology
- Maximize what they can do
- Baclofen pump
S/S for cerebral palsy
increased or decreased muscle tone, gross and fine motor delays, feeding difficulties, seizures, joint deformities
Club foot treatment
- Nonsurgical
- Serial casting with long leg casts changed weekly
- Brace for 2-4yrs after tx to retain
- Surgical
How long should an individual with club foot expect to wear a brace after tx?
2-4yrs
What does serial casting do?
Straighten it out more and more each week
Cast care teaching plan
- Keep cast clean and dry - Put nothing inside of cast
What kind of toy do you give a child who has a cast
Something that is too big to fit inside of cast
What activities should a child who has legg calve perthes avoid?
No running, jumping or other high-impact activities that might accelerate hip damage
What additional cast assessments are necessary besides 5 Ps?
Bleeding, signs of infection, skin breakdown
