Neuromuscular Abnormalities Flashcards
Phoria
eyes deviate when fusion is broken
Tropia
manifest deviation of the eyes
Unilateral Tropia
patient fixates with one eye
Constant Tropia
fusion is not enough to keep eyes aligned
Intermittent Tropia
fusion functions at times, but not always
Norms for CT
Distance 1 XP (+/- 2)
Near 3 XP (+/- 3)
Neutralizing Exo
Base In
Neutralizing Eso
Base Out
Neutralizing Hyper
Base Down
Neutralizing Hypo
Base Up
Splitting Vertical Prism
absolute sum of prism
keep same base
OD 4BU / OS 3 BD = 7 OD BU or 7 OS BD
Splitting Horizontal Prism
keep same base OU
8 BO = 4 BO OD / 4 BO OS
Torticollis
abnormal head position
Ocular Torticollis
compensatory response to ocular problems to maintain binocularity and improve VAs
Abnormalities that cause Ocular Torticollis
nystagmus strabismus supranuclear disorders monocular blindness/ ptosis refractive error visual field defect
Comitant
deviation size remains same or within 5pd in all positions of gaze
no muscles are over or underacting
Non-Comitant
deviations different in different positions of gaze
over or under action of one or more muscles
Congenital/ Acquired Strabismus Timeline
before/ after 6 months
Congenital History
uncertain onset
intermittent diplopia or asymptomatic
Aquired History
exact time of onset is known
Congenital Head Posture
family is unaware of abnormal posture
Acquire Head Posture
patient is aware of posturing
Congenital Comitancy
appears comitant because of developed muscle sequalea
Acquired Comitancy
over action of yoked muscle seen
Paralysis
action of muscle is completely
Paresis
action of muscle is impaired
Palsy
general term of paresis/ paralysis
Neurogenic Deviation
apparent deviation in primary gaze
Mechanical Deviation
little/ no deviation in primary gaze
Neurogenic Diplopia
same in different gazes
Mechanical Diplopia
opposite to gaze
Neurogenic Ocular Movement
greater movements on ductions than versions
Mechanical Ocular Movement
duction and version movement equally limited
Neurogenic Hess Lancaster
field of affected eye is smaller and displaced equal to deviation
Mechanical Hess Lancaster
fields are irregular and close to each other
Limits phoria symptoms
sensorimotor system
Deviations more likely to cause symptoms
vertical deviations
Poor fusion is associated with……
fatigue asthenopia headaches diplopia suppression avoidance
