Management of acute myelopathy

A
  1. Steroid (reduce swelling/infammation)
  2. Plasmapherisis (remove antibody&plasma in autoimmune esp)
  3. IV immunoglobulin (reset immune system)
  4. Analgesics
  5. Antiviral (if infection)
  6. Rehab
  7. Education

A
  1. Progressive weakness in both arms and legs
  2. Areflexia
  3. Others: symmetrical, > 4weeks, cranial nerve involvement, autonomic dysfunction, high protein in CSF

Autonomic dys: Tachy, sweating, BP chg

A

General

  1. Secure ABC
  2. Keep NBM: NG tube
  3. Vital signs
  4. Ventilation
  5. DVT prophylaxis
  6. Chest physiotherapy
  7. Urinary catherizaiton
  8. Analgesic

Specific

  1. IV immuglobulin within 14 days
