contralateral weakness in the lower extremity, medial leminiscus - (decreased contralateral proprioception), ipsilateral paralysis of the hypoglossal nerve
Medial Medullary Syndrome - Anterior spinal artery
loss of pain and temperature sensation on the contralateral side of the body and ipsilateral side of the face, ipsilateral Horners, ipsilateral ataxia, trigeminal nucleus (spinal tract and nucleus)
PICA - lateral medullary syndrome aka Wallenburgs Syndrome
ipsilateral facial paralysis, ipsilateral loss of lacrimation and reduced salivation, ipsilateral loss of taste from anterior 2/3 of the tongue, ipsilateral cochlear nucleus, vestibular (nystagmus), ipsilateral facial pain and temperature ipsilateral dystaxia (MCP, ICP)
AICA - lateral inferior pontine syndrome
Contralateral homonymous hemianopia with macular sparing; supplies occipital cortex
Posterior cerebral artery
Contralateral face and arm paralysis and sensory loss; aphasia (dominant sphere), left sided neglect
Middle cerebral artery
Most common site of circle of willis aneurysms - lesion may cause visual defects
Anterior communicating artery
Common area of aneurysms - causes CN III palsy (oculomotor problems); pupil involved and eye is down and out
Posterior communicating artery
Patient who is extremely hypotensive develops upper leg, upper arm weakness, defects in higher order visual processing
Watershed Zones - T4, L1
Locked in syndrome - CN III intact
Basilar Artery
general sensory and motor dysfunction; aphasia
In general, stroke of anterior circle
CN defects, (vertigo, visual defects), coma, cerebellar deficits (ataxia). Dominant hemisphere (ataxia), non dominant (neglect)
In general, stroke of posterior circle
Ehlors Danlos, Marfans Syndrome, Adult Polycystic Kidney Disease
associated with aneurysm
