A person with Parkinson’s who is struggling with their mobility
A person with traumatic brain injury who was a chest infection
A person with a spinal cord injury who is having trouble transferring

What questions would you ask in your HPC?

A

When did the stroke occur?
What has happened since their stroke?
How did they fall? How long have they had MS? How has the Parkinson’s progressed over time? Where were the initial symptoms?
How were they treated?
What tests have been done?
Have they had physio before and did it help?

What questions would you ask in your PMH?

A

Neurological systems: any previous strokes or other neurological events
Musculoskeletal system : osteoarthritis Staios fractures soft tissue, injuries or pain anywhere else
Respiratory system : chronic COPD asthma, acute pneumonia or any surgery
Cardiovascular system : NA myocardial infarction, congestive cardiac failure or any surgery
Urinary system : kidney disease
Endocrine system : any diabetes, problems with their hormones, thyroid issues

Special questions for neuro

A

Falls - have you fallen? How often? What was the cause? Did you loose consciousness? Are they worried about falling? Did they hurt themselves? Use a falls confidence scale to measure this.

Fatigue - do they get excessive fatigue? What makes it worse? What makes it better? Are they sleeping? How is their diet? Do they feel stressed? Do they exercise? Use a fatigue scale to measure this.

Pain - do they have any pain? Where is it? What type of pain is it? SIN FACTOR? Use a pain rating scale to measure this.

A

Speech - expressive and receptive dysphasia
Eyesight - glasses or homonymous hemianopia
Hearing - hearing aid
Cognition - following instructions, alertness and orientated
Consciousness
English language speaking
