What would you ask in the HPC
History of the problems and symptoms
How long ago did they start?
How did they start where they sudden onset or gradually overtime?
What was the cause of the symptoms?
If there’s more than one problem, what is the relationship between the symptoms? Which one started first do the problems or symptoms occur together or separately?
What would you ask in your PMH?
Any medical history relevant to the problem?
History of previous episodes
Previous treatment if relevant
Ask about THREADS, systems
What would you ask in the SH & FH
Their age
Employment status
Do they smoke or drink alcohol?
Their home situation
If they have any hobbies
Do they drive?
Any genetic conditions
What would you ask in the DH?
Are they on any medication?
Any recent changes in their medication
How often do they take their medication and what dosage?
Any allergies
Do they use self administered drugs?
