CN I - Olfactory : Smell
CN II - Optic : sight seeing
CN III - Oculomotor : eyelids pupil constriction CN IV - Trochlear : eye downward /inward
CN V - Trigeminal : sharp dull cheek feeling jaw
CN VI - abducens : eye laterally
CN VII - Facial : smile frown puff cheeks 2/3 anterior tongue
CN VIII - vestibulochocoler (acoustic): hearing
CN IX - glossopharyngeal : gag reflex swallowing 1/3 posterior tongue
CN X - Vagus : swallowing speaking
CN XI - Accessory: shoulder movement
CN XII - Hypoglossal : movement of tongue + strength

  1. Palpation/ sensation
    • light touch; sharp touch; dull; vibratory
    • 2-point discrimination (apply two points of an open paper clip lightly to the skin in ever-closing distances. Note when no longer perceive two separate points)
  2. Assess muscle strength
  3. Assess coordination (finger-to-nose Test) (lying heel-to-shin test)
  4. Assess sense of balance and position Romberg Test
