Olfactory
Sense of smell
Often affected by rhinitis
Anosmia (loss of sense of smell) early sign of Parkinson’s (can they smell coffee)
II
2 optic
Visual
Some strokes may affect vision
Snellen chart
III oculomotor 3
IV trochlear 4
VI abducens 6
Help move eyes so assessed together
Can eyes follow pen
V trigrminal 5
Sensory perception of face, sinuses and teeth
Involved in muscle that control mastication(chewing)
Can they feel cotton swab on face with eyes closed on both sides
Can they clench their teeth
VII facial 7
Control facial muscles of expression
Sensory expression of tongue
Can they smile symmetrical or raise eyebrows
Stroke may effect this
VIII vestibulocochlear 8
Aids in equilibrium and hearing
Does patient have vertigo, unsteady gait or any auditory dysfunction
Stroke affect this
IX glossopharyngeal 9
Aids in taste sensation and movement of pharyngeal muscles
Test gag reflex
Have patients say “ahh”
X vagus 10
Senses much of the viscera from thorax to abdomen
XI accessory 11
Involved with movement or the sternocleidomastoid and trapezius muscles
Ask pt to shrug shoulders- should be straight and symmetrical
XII hypoglossal 12
Motor function of tongue
Ask pt to protrude tongue which should be midline; ask to swish side to side
