Neuroleptic malignant syndrome, SS syndrome Flashcards
NMS onset
Longer than SS - 1-7 days
What do both NMS and SS cause
Severe autonomic dysfunction -> hyperthermia, HPTN, tachycarida
Neuro abnoramlities
Who is often predisposed to NMS
Lewy body dementia patients - v sensitive to antipsychotics
What movements is NMS ass with
Hyperreflexia, lead pipe rigidity
Symptoms of serotnin syndrome
Abrupt <1 dya
Hyperreflexia
Increased tone and clonus
Dilated pupils
Hyperactive bowel sounds
Serotonergic drugs eg SSRIs, SNRIs, tramadol, cocaine
Cause of NMS
Dopamine antagonists eg anti-psychotics, metoclopramide
OR sudden stopping of dopamineic drugs eg levodopa
Medical Mangement of NMS if severe
Bromocritpine - dopamine agonist + dantrolene (muscle relaxant)
Medical mangement Serotonin syndrome
Benzodiazapines
Supportive management for NMS and SS
Stop offending drug
IV fluids, cooling, addressing any further metabolic or electolyte abnormalitites
Risk factors for NMS/SS
Rapid titration
Dehydraton
Concurrent illness Prev episodes
Gentetuc
Investigations for NMS
Raised CK
AKI if severe
Leukocytosis
