most common cause of TIA/CVA
emboli
signs and sx of subclavian steal syndrome
sx= vertebrobasila arterial insufficiency (n/v, double vision, vertigo, dysphagia etc)
signs= BP in left are less than right
presentation of MCA stroke
contralateral weakness
sensory loss
hyperreflexia
name 4 causes of stroke
- emboli - most common
- thrombotic - think large vessels like MCA and carotids
- lacunar- think HTN, cause by thickening of dz vessel wall
- nonvascular - anoxic brain injury
clinical features of lacunar stroke
pure motor or pure sensory
- motor –> internal capsule
- sensory –> thalamus
cerebral edema post infarct
appears in 1-2 days, can cause mass effect for up to 10
tx with hyperventilation and mannitol
saccular (berry) aneuryms are found…
at the bifurcation of arteries of circle of willis
diagnosis of SAH gold standard?
Xanthochromia (yellow CSF) on LP
*blood in CSF is hallmark
xanthochromia
yellow CSF seen in SAH is indication of RBC lysis and that blood is NOT from a traumatic spinal tap
SAH is diagnosed. next step?
angiogram –> for surgical clipping
