Neuro Primary Survey
A
- pt is GCS 15 and alert
- not collapsed or fallen
- No signs of trauma
- no obvious distress or pain

A
- headache
- dizziness/syncope
- seizure activity
- numbness/weakness
- balance/memory
- vision/hearing
- head injury

Vision/Hearing Questioning
A
- any changes in vision? = dmg to eyes
- loss of sight/blurriness = migraines
- any changes in hearing?
- do you have hearing aids?

Head Injury Questioning
A
- any recent trauma?
- If there was a decrease in GCS, I’d be thinking about Cushing atriad
- Red flags for increased intracranial pressure = high BP, Brady, irregular resps
- Head Injury Red Flags - > vomiting syncope, amnesia, headache, blood thinners

Balance and Memory Questioning
A
- do you feel unbalanced? = ear infection
- vertigo = feeling like the room is spinning around you = labyrinthitis (balance, vertigo, tinnitus. Ear infection)
= vestibular neuritis = ear infection without tinnitus - any changes to memory = dementia, clot, tumour

Numbness and Weakness Questioning
A
- have you had any numbness in limbs or other areas?
- any muscle weakness?

Seizure Activity Questioning
A
- have you had any seizures?
- are you epileptic? = could be psychogenic

Dizziness and Syncope Questioning
A
- dizziness (not light headed) = anaemia, hypokolaemia, migraine
- any syncope? = lack of blood flow

Headache Questions
A
- do you suffer from headaches?
- does anything trigger them for you?
- do you get migraines?
Thunderclap - sudden onset (meningitis, sub-arach, encephalitis - think primary, secondary headache
- headache for over two weeks needs investigating

Neuro PMH
A
- previous strokes/tax?
- do you suffer with migraines?
- do you take regular medication eg epilepsy Med adherence
-do you have any allergies? - do you use recreational drugs? (opiate use)
- common conditions include, epilepsy, tinnitus, diabetes (neuropathy), HTN, bell’s palsey neuralgia
