2. Ask person, place, time and situation
3. CN exam 2-12
4. Sensation upper and lower bilaterally
5. BILATERAL Bicep and Patella DTR
6. Rapid alternating movement: cerebellar exam
7. BILATERAL babinski test
8. Gait (normak walk, tandem walk, walk on heels)

“What is your full name”
“Where are we”
“What is the date”
“What are we doing right now”

Question 3

3. CN 2-12 exam

Answer 3

• CN 2/3: Look at direct and consensual pupillary constriction
• CN 3,4 and 6: H tests
o Make to do convergence
• CN 5: ask them to move jaw back and forth against resistance
o Sensory: cotton ball on face- dull and sharp part
 Access V1, V2 and V3
• CN 7: raise eyebrows, close eyes tight
• CN 8: stand behind patient, have them occlude opposite ear and whisper from a small distance and tell them to repeat what you say.
• CN 9/10: test gag reflex with a tongue depressor
o Ask patient to swallow: no drooling and can swallow so good
• CN 11: Ask patient to shrug against resistance
• CN 12: Push tongue against cheek

4. Evaluate sensation upper and lower bilaterally (pain and/or vibratory sensation) 

Answer 4

• Tell patient to close eyes
• Prick patient on both arms: ask where they felt the prick
• Prick patient on both legs: ask where they felt the prick

Question 5

Babinski

Answer 5

• Babinski reflex: use bottom of reflex hammer and slide on bottom of foot
o – response: toes curl forward
o + response: toes flair out
o DO BILATERALLY
