Mental Status Examination: 5 Components
- Appearance and behavior
- Mood and affect
- Speech and thought processes
- Thought content and perceptual abnormalities
- Sensorial, cognitive, and intellectual functioning
CN I
Olfactory (sensory)
*smell
- Test non irritating smells
CN II, III
Optic and Oculomotor
- Visual Acuity (sensory)
- visual fields, ocular fundi
- pupillary reactions

1. Test visual fields by confrontation
II & III
- size and shape of pupils
- pupillary reactions to light
- Near response (pupillary constriction: pupillary constrictor muscle, convergence: medial rectus muscles, accommodation: ciliary muscle)
CN III, IV, VI
III. Oculomotor (motor)
*pupillary reactions
IV. Trochlear (motor)
VI. Abducens (motor)
*extraocular movements (6 cardinal directions)
NOTE: diplopia, nystagmus, ptosis
CN V
Trigeminal (motor)
- corneal reflexes, facial sensation, jaw movements
- voice
- palpate temporal and masseter muscles
- sharp/dull on forehead, cheeks, chin
- corneal reflex (not appropriate if contact lenses in place)
CN VII
Facial (motor/sensory)
- facial movements
- voice
- Raise eyebrows
- Frown
- Close eyes tightly and resist opening
- Show upper/lower teeth
- Smile
- Puff out both cheeks
CN VIII
Acoustic and Vestibular (sensory)
*hearing
- whisper test
- if positive, Rinne (air and bone conduction) and Weber (lateralization) tests)
CN IX, X
IX. Glossopharyngeal (sensory/motor)
X. Vagus (sensory/motor)
*swallowing & rise of palate, gag reflex
*voice & speech
- Note voice quality
- Swallowing difficulty?
- Say “ah” & note movement of soft palate and uvula
- Gag reflex if patient is willing
CN XI
Spinal Accessory (motor) *shoulder/neck movements
- Shrug shoulders
- Rotate neck against resistance, both directions
CN XII
Hypoglossal (motor)
*tongue symmetry, position, movement
- Word articulation
- Protrude tongue
- Move tongue side to side
- If ambiguous, push tongue against inside of each cheek as you palpate each cheek
Brudzinski Sign
Flex neck. Flexion of hips and knees is positive.
Kerney Sign
Flex patient’s leg at hip and knee, then extend leg to straighten knee.
Pain and increased resistance to extension are positive. Discomfort is normal.
Nuchal Ridigity
Flex neck forward using hands behind head until chin touches chest
Neck normal supple
Straight Leg Test
Evaluates for lumbosacral radiculopathy
Raise patient’s relaxed straightened leg
Positive if pain radiates into ipsilateral leg
