Nephrotoxicity Flashcards
Name two forms of kidney impairment
Acute kidney injury (AKI) and chronic kidney disease (CKD)
CKD - age
aging can be a cause of renal impairment
What is the prognosis for CKD
Check both GFR and ACR - and find on the scale
What is the prognosis for CKD
Check both GFR and ACR - and find on the scale
AKI - what is the definition
abrupt decrease in kidney function that includes but is not limited to acute renal failure.
This is why we don’t use GFR for AKI 0 it needs to be in a steady state (there is a dramatic change)
List the criteria of AKI
Increase in serum creatinine
Increase in SCr
Urine volume is lower than normal
Decrease in eGFR
What is KDIGO classification
General markers for the stage of AKI - assesses serum creatinine and urine output
Does depend on drug, exposure and time etc.
List biomarkers for kidney toxicology
Urinary proteins w enzymatic activity Proteinuria Kidney injury molecule 1 (KIM-1) Neutrophil gelatinase-associated lipocalin Cytokines Clusterin Type IV collagen Osteopontin
Give examples of pre-renal toxicology
Reduced perfusion
Volume depletion
Altered renal haemodynamics
Give examples of intrinsic nephrotoxicity
Glomerulonephritis
actue tubular necrosis
actue interstitial nephritis
Post-renal (obstructive) nephrotoxicity
Ureteric/retroperitoneal fibrosis
renal calculi
thrombotic microangiopathy
Vasoconstriction and dilation - drugs with renal impairment
ACE, NSAIDs, loop diuretics
