Nephrology Flashcards
Diagnosis: waxy casts?
Chronic kidney disease
Diagnosis: leukocyte esterase in urinalysis?
Suggests UTI
What is delta gap?
Anion gap minus 12 plus the patient’s HCO3
What does delta gap of 24 signify?
There is concommitant metabolic acidosis
Diagnosis: glomerular, “muddy brown” casts?
Acute tubular necrosis (ATN)
What anion gap suggests metabolic acidosis?
Anion gap =>20
What does a delta gap > 30 signify?
Concommittant metabolic alkalosis
Identify the primary disorder: pH > 7.4, HCO3 > 24
Metabolic alkalosis
What are the causes of gap acidosis?
Use the mnemonic MUDPILERS
M=methanol
D=diabetic acidosis
P=paraldehyde
I=INH, Iron
L=Lactic acidosis
E=ethylene glycol
R=rhabdomyolysis
S=salicylates
How do you determine the anion gap?
Sodium minus HCO3 and chloride
Diagnosis: WBC casts?
Pyelonephritis or
Interstitial nephritis
What is the normal anion gap?
12
Identify the primary disorder: pH > 7.40, pCO2 40
Respiratory alkalosis
Diagnosis: Nitrate in urinalysis?
Gram - bacteriuria
What are RBC casts and dysmorphic RBCs associated with?
Glomerulonephritis
