What will PE show in Nephrotic Syndrome?
Pitting edema (peripheral and periorbital)
Ambulatory pt will have edema in peripheral areas
Non-ambulatory pt will have edema around the sacrum b/c fluid is influenced by posture
What will labs show in Nephrotic Syndrome?
Proteinuria > 3.5g/24hr
Hyperlipidemia
Hypercoagulability (renal vein thrombosis)
fatty casts
What is the mechanism of action of Tamsulosin and the “zosin” drugs?
alpha-1 adrenergic antagonists: relax smooth muscle in the bladder neck, prostatic capsule and prostatic urethra.
should be administered immediately at the time of catheterization to prevent the recurrence of early acute urinary retention due to benign prostatic hyperplasia (BPH).
When should you consider surgery in a pt with BPH?
If a pt with BPH fails two voiding trials
What is the initial dx of testicular cancer?
trans-scrotal US
An ultrasound will confirm the presence of a mass and rule out benign processes such as hydrocele or epididymitis
What is the MC presentation of testicular cancer?
painless testicular mass
What is the most common type of testicular cancer?
seminomatous germ cell tumor
What will labs show in testicular cancer?
beta-hCG, AFP or LDH (tumor markers)
MC risk factor for testicular CA?
cryptorchidism
What is the tx for testicular CA?
radical orchiectomy
