Definition
intracranial haemorrhage originating in periventricular subependymal area with blood then into ventricular system
early IVH
within 72hrs
late IVH
after 72hrs
incidence
30% infants <1500g
pathophysiology
unsupported blood vessels in subependymal germinal matrix
instability of bp associated with birth trauma and resp distress
what may decrease incidence in prems?
delayed clamping cord
Risk factors
PREMATURITY hypertension coagulopathies rapid volume expansion anaemia, vacuum, acidosis
clinical features
asymptomatic
apnoea, bradycardia, acidosis, cutaneous mottling, bulging fontanelle, high pitched cry
absent moro reflex
seizures
large IVH features
rapid onset coma
seizures
pupils fixed to light
diagnosis
USS and MRI
how many grades?
4
antenatal prevention
avoid preterm delivery
vitamin K to those on phenytoin
c section high risk
antenatal CCS
postnatal prevention
avoid birth asphyxia
avoid rapid infusion, excess handling
correct acid base and coagulation
complications
posthaemorrhagic hydrocephalus
seizures
irreversible brain damage
death
