Necrotising Gingivitis/Periodontits Flashcards
What is Necrotising Gingivitis
A severe inflammatory periodontal disease caused by plaque bacteria that is rapidly destructive and debilitating
What does ‘NG is an opportunistic infection’ mean
It occurs more often and is more severe in people with weakened immune systems than in people with healthy immune systems
It inhabits a healthy oral cavity
What is Necrotising Ulcerative Gingivitis
A common non-contagious infection of the gingiva
What is Necrotising Ulcerative Periodontitis
An infection of the gingiva which leads to attachment loss
What is necrotising stomatitis?
Bone denudation extended through the alveolar mucosa
It causes larger areas of osteitis and bone sequestrum
When does Necrotising Stomatitis occur
when NUP progresses into tissues beyond the mucogingival junction
What groups of people are at greater risk of Necrotising Stomatitis
People with HIV
People with malnutrition
What is Cancrum Oris
A necrotising and destructive infection of the face and mouth (affects mucous membranes)
Signs of NUG/NUP
- ulcerated and necrotic papillae that cause a punched out appearance of gingiva
- Ulcers covered in yellow, grey or white slough/slime
What is in the slough covering the ulcers
- fibrin
- necrotic tissue
- leucocytes
- erythrocytes
- mass of bacteria
What are symptoms of NUG/NUP
-bleeding that occurs spontaneously
- painful gingiva
- Swelling of lymph nodes
- Halitosis - bad breath
- gingiva looking grey in colour
What bacteria is associated with NUG/NUP
- Treponema sp.
- Selenomonas sp.
- Fusobacterium sp.
- Prevotella intermedia
What are risk factors of NUG/NUP
-stress
- smoking
- immunosuppression
- malnutrition
-sleep deprivation
- poor oral hygiene
What are 5 treatment options
- Ultrasonic debridement
- 0.2% chlorhexidine mouth 2x daily
- Antibiotics for patients with malaise and fever
- Smoking cessation, vitamin supplementation, dietary advice
- Hygiene phase therapy if NUP
Why is chlorhexidine mouthwash used
It is an antiseptic and disinfectant used if the pain is preventing the patient from brushing
