Classification of Periodontal Disease Flashcards
What are the 4 components of classification
- Stage
- Grade
- Stability
- Risk Factors
What does staging measure?
the disease severity
How do you determine the stage of periodontal disease
assess the amount of bone loss at the worst affected site
- coronal third, middle third, apical third
What does grade measure?
Susceptibility and speed of progression
How do you determine the grade of periodontitis?
amount of bone loss (%) divided by the patients age
What are the 3 diagnosis for BPE codes 0,1,2 and how are they determined
Gingival health - <10% BOP
Localised gingivitis- 10-30% BOP
Generalised gingivitis - >30% BOP
What is the response to BPE 3
Initial therapy - OHI and PMPR
Review in 3 months and do 6 point pocket chart of that sextant
What are the diagnosis for BPE code 4 and how are they determined
Periodontitis Molar Incisor Pattern - molar incisor pattern
Localised Periodontitis - <30% of teeth
Generalised Periodontitis >30% of teeth
What are the 4 stages when assessing bone loss
Stage 1 - <2mm attachment loss from CEJ
Stage 2 - Coronal third of the root
Stage 3 - Mid third of the root
Stage 4 - Apical third of the root
What are the 3 grades when assessing bone loss
BONE LOSS(%) / PATIENT’S AGE
Grade A - <0.5
Grade B - 0.5-1
Grade C - >1
What is the criteria for ‘currently stable’ periodontitis status
BOP <10%
PPD </=to 4mm
No BOP at 4mm sites
What is the criteria for ‘currently in remission’ periodontitis status
BOP >/= 10%
PPD </= 4mm
No BOP at 4mm sites
What is the criteria for ‘currently unstable’ periodontitis status
PPD >/= 5mm
OR
PPD >/=4mm and BOP
What are risk factors for periodontitis
Smoking
Sub optimally controlled diabetes
Pregnancy
Medication (e.g. calcium channel blockers, phenytoin)
Family history
Cardiovascular disease
Stress
Diet
RA
Why is smoking a risk factor
- reduces gingival blood flow
- impairs wound healing
- increases production of inflammation-mediating cytokines
