Neck pain Flashcards
1
Q
Main causes of neck pain
A
- Vertebral dysfunction - faset joint
- Strain/sprain of ligaments
Red flag points similar to low back pain
2
Q
Brachial plexus parts
A
RhTDm(C)
Roots - C5 to C8, T1
Trunk - Upper, middle, lower
Divisions - Anterior, posterior
Cords - lateral, posterior, medial
3
Q
Brachial plexus associated reflexes
A
- Bicep jerk: C5>C6
- Brachioradial: C6>C5
- Triceps: C7
- Finger: C8
4
Q
Brachial plexus associated functions
A
C5: arm abd
C6: Elbow flexion, wrist extension
C7: Wrist flexion, elbow&finger extension
C8: Grip
5
Q
Cervical spondylosis
A
- Degeneration of cervical vertebra
- mc C5-6, C6-7
- Narrowing of intervertebral foramen (spinal nerve emerge) … immature bone formation .. radiculopathy
- Dull aching pain, worse in morning after lifting head
- Improves with mild activity, deteriorates with heavy activity, aggravated by flexion and extension
- Pain radiates to head, arm, scapula
- A/w vertigo
- Xray - osteoarthritic wear and tear of vertebra (not needed)
- Mx: PCM, NSAID, exercise
6
Q
Complications of cervical spondylosis
A
- Radiculopathy - compression of nerve roots
- Myelopathy - compression of spinal cord (best inv - MRI)
- Spinal canal stenosis
7
Q
Radiculopathy
A
- Sudden onset (sudden neck movement)
- Radiates to arm
- Paraesthesia in *hand, forearm
- Xray not req until 6-8wks (unless red flag+)
- Rx: rest, analgesic
8
Q
Myelopathy
A
- Insidious onset (yrs)
- Numbness, tingling in fingers
- Leg stiffness (both UL and LL affected)
- UMN lesion signs:
Spastic weakness
↑Tone
Hyper-reflexia - Urgent MRI
- CT with myelogram - most accurate
