NB at risk 2 Flashcards
1
Q
preterm NB resp prob
A
- not enough surfectant
2. pulm vesseles not dev/can’t constrict -> L to R shunt of blood through ductus arteriosus, may remain open
2
Q
preterm NB resp complications-4
A
pulm congestion, increased resp effort, CO2 retention, bounding femoral pulse
3
Q
preterm NB thermoregulation-6
A
- high ratio of body surface to body wt decreases ability to produce heat
- little subcut fat
- thin skin raises insensible heat loss
- less flex of extremities (extend at 28 wks)
- low vasoconstriction ability
- low brown fat and glycogen in liver
4
Q
preterm NB GI-7
A
- high risk aspiration- poor gag, undeveloped sphincters, poor suck/swallow
- unmet nutrition sm stomach
- limited conversion to nonessential amino acids
- undev kidneys
- lactose/sat fat absorption limited
- deficient in Ca and phos-ricketts
- high basal metab rate increase O2 needs
5
Q
preterm NB renal-4
A
- low GFR before 34 wks
- gradual decline in UOP r/t decline in BP
- limited response to ADH- fl imbalance
- affects drug excretion
6
Q
preterm NB immune-3
A
- passive immunity in 3T (IgG)
- IgA in breast milk
- bacterial sepsis- sudden onset of apnea and bradycardia
7
Q
preterm NB neuro-3
A
- rapid brain development in 3T
- period of reactivity delayed
- sleep-wake disorganized
8
Q
preterm NB nutrition-4
A
- early feed- 95-130 cal/kg/day
- need wt gain 20-30g day
- oral feed ready- hands to mouth, alert, suck, root
- BF ready- suck/swallow, consistent wt gain, control temp
9
Q
gavage-5
A
lack suck/swallow, vent, orogastric preferred over nasal
- check residual in stomach first
- after feed on rt side or belly
10
Q
preterm NB complications-7
A
- apnea of prematurity-20 sec + or w cyanosis, pallor, brady (day 2-7)
- patent ductus arteriosus
- respiratory distress syn (surfectant)
- intraventricular hem (less 34 wks, longterm comp)
- retinopathy of prematurity
- bronchopulmonary dysplasia
- speech/ neuro/ auditory defects
11
Q
developmental progress
A
evaluated based on expected DOB not actual DOB
12
Q
preterm NB ass 1-5
A
- color- pink, ruddy, cyanotic
- skin- red, translucent, see veins, no subcut fat
- lanugo plentiful
- head size large in relation to body
- skull bones pliable w flat fontanelles
13
Q
preterm NB ass 6-11
A
- ears pliable fold over
- genitals non ruggated, prominent clitoris and minora
- posture flaccid, frog-like
- weak feeble cry
- reflex- poor suck/swallow/gag, incomplete moro
- activity jerky low tone
