NAVOSH 5-3-2 Hearing Conservation Data Flashcards
Who manages the Hearing Conservation Program?
The IDC
Who establishes the Hearing Conservation program (HCP) in the command?
The CO
Who is the liaison to the IH Dept, and Occ Audiologist and conducts noise measurement and exposure analysis?
Safety Officer
What records does the Safety Officer maintain?
Noise hazardous areas
Noise hazardous equipment
Baseline and subsequent (IH Surveys)
How often is the HCP evaluated?
Annually
Who annually certifies the audio metric testing booths?
IH Officer
What does the MDR do regarding HCP?
Conducts training for all hands initially and annually
Reports to the safety officer the permanent threshold of deteriorated hearing
-Name, rank, rate, work center, time onboard
_____ ________ are taken as part of the IH Survey.
Noise measurements
NAVMED 6260/2
Hazardous Noise Warning decal 8” x 10”
NAVMED 6260/2A
Hazardous noise label 2” x 2”
Where are hazardous noise labels placed, given what exemption?
Posted outside doors and hatches except if it leads to the weather deck then it will be posted inside the door or hatch
What is Noise Abatement?
Reduction of noise at the source before implementing PPE
What decibels are labeled as Noise hazardous and require use of double ear pro?
Noise levels >96 dBA or >165 dBP
Will personnel exposed to gunfire and weapon systems wear hearing protection?
Yes, obviously
What are the Hearing testing and medical surveillance?
Reference Hearing Test
Monitoring Hearing Test
Termination Hearing Test
What are the 2 forms associated with HCP?
DD 2215- Baseline Audiogram
DD 2216- Monitoring Audiogram
If noise levels are less than or equal to 84dBA then…
No hearing protection required
If noise levels are greater than or equal to 85dBA but less than 96 dBA then…
Single hearing pro required
If Noise levels equal to 96 dBA or greater then…
Double hearing protection required
IHO and Occ Audiologist take measurements at what ranges?
Noise levels greater than or equal to 85 dB (continuous or intermittent) or 140 dB peak sound level of impact or impulse noise
When do the IHO and Occ Audiologist resurvey?
30 days after any significant modification or changes in routine.
How long are the records kept for and by whom?
50 years and kept by the measuring activity.
(IHO or Occupational Audiologist)
What is a significant threshold shift (STS)?
A change in hearing threshold relative to the current Audiogram of an average of 10 dB or more at 2000, 3000, and 4000 Hz in either ear.
If the STS is positive, the hearing is
Poorer
If the STS is negative, the hearing is
Better than reference
Procedure If the STS is -
Retest the same day
If still - assume DD 2215 is wrong and establish new reference Audiogram
Procedure If the STS is +
Retest after 14 hours “Noise free”
Notify Supervisor
Procedure if retest of positive STS does not indicate STS…
Return to annual monitoring
Procedure if the retest of a positive STS showed STS w/ frequencies below 3000 HZ
Screen for medical issues
Record in SF600
If abnormal: health care evaluation must be obtained and must be medically cleared
If Normal: perform second retest
What is the procedure for the second retest of a positive STS?
Can be administered on same day
If no STS indicated: return to normal monitoring
If STS indicated: hearing protection is evaluated, results forwarded to Audiologist or physician for review
Results will typically be used to re-establish reference Audiogram
How many days does the supervisor and worker have to be notified of positive STS?
21 days
Where is the positive STS noted
OSH office and entry in OPNAV 5102/7 (log of navy injuries and occupational illness)
What’s the purpose of the noise free Audiogram?
Physical exclusion period from noise is “auditory rest” for 14 hours to return to pre-exposure levels
What are noise abatement strategies?
Engineering
Administration
What are the types of personal hearing protective devices?
Ear plugs: disposable, single/double/triple flange
Straightaway muffs:
Circumaural muff
