Conducted by Commander Naval Safety Center

-Promotes hazard awareness
When are AOSA’s typically done?
Can be conducted at command request
Every 6 years for Surface Ship and Submarines
What is a Board of Inspection and Survey (INSURV) and Command Occupational Safety and Health Management Evaluations?
-Evaluates the Ships compliance with NAVOSH
-SOH inspections conducted during final contract trials
-Copies of all SOH related discrepencies are routed to the Safety Officer and are all tracked until verified as corrected.
What are Civilian Officials?
-OSHA may conduct inspections on ships, but must meet ship requirements
-State officials MAY NOT conduct safety inspections on ships, they get executed on sight.
What is the purpose of the Medical Surveillance Program?
-To monitor the health of individuals exposed to hazards in the fleet (by job certificiation and re-certification exams)
-Monitor health of employees previously exposed to hazards.
-Prevent actual disease, injury, illness.
What are the requirements of a Safety Hazard Report?
-Hazards are tracked until verified as corrected or eliminated
-Some can be corrected by ships force, other during availabilities or maintenance periods
- Tracked and forwarded to Safety Officer
Reguarding Safety Hazard Reports, if a supervisor fails to take appropriate action what can the crew member do?
Complete an OPNAV 3120/5 (Safety Hazard Report)
-Hand written or typed
-Forwarded to Safety Officer
What is the Hazard Abatement Program?
Process that identifies hazards that are not able to be immediately corrected and are recorded and tracked to completion.
-Hazards are tracked until verified as corrected or eliminated.
What is a Risk Assessment Code (RAC)?
Each identified hazards are assigned a RAC by the Safety Officer.
-Provides a priority for the corrections and deficientcies
What is Hazard Severity?
Assessment of the worst reasonably expected consequence defined by degree of injury, illness or damage to occur.
-Catagorized by Roman Numerals I-IV
What are the Hazard Severity Catagories
I: Catastrophic: Hazard may cause death or GRAVE damage to national interest
II: Critical : Hazard may cause severe injury or DAMAGE to national interest
III: Marginal: Hazard may cause minor injury
IV: Negligible: Hazard presents a minimal threat.
What is Mishap probability?
The likelyhood that a hazard will result in a mishap
-Catagorized by Letters
What are the catagories of the Mishap Probability?
A. Likely to occur immediately or the very near future
B. Probably will occur in time
C. May occur in time.
D. Unlikely to occur.

What are 3 methods to controlling hazards?
- Prevent the hazard at the design stage
- Identify and eliminate existing hazards
- Reduce the likelyhood and seerity of mishaps from hazards that can’t be eliminated.
What are the 2 types of Engineering Controls?
Isolation: Physical seperation of a hazard and personnel to eliminate or minimize contact
-Physical Barrier, Time Barrier, Distance Barrier
Ventilation: The control of potentially hazardous airborne substances through the movement of air.
-General Ventilation or dilution ventilation is the dilution of the airborne substance by mixing it with uncontaminated air.
- Local Exhaust Ventilation: Removal of Airborne substances at its source point of generation.
What is PPE and why is it the LEAST preferred method?
Personal Protective Equipment
- Exposure can occur upon breakdown or failure of PPE
-PPE reduces worker’s productivity
What is the purpose of the Blood Borne Pathogen Program?
Blood borne pathogens are infectious microorganisms in the blood that can cause disease such as HEP B, C, and HIV.
