nausea and emesis Flashcards
where is the chemo receptor trigger zone (CTZ) located and what does it do?
outside of BBB, senses what is going on in the blood
recognises emetic agents (toxins, drugs)
where is the vomiting centre located and what does it control?
in medulla oblongata - fourth ventricle
induces act of vomiting from pain, smells, motion
what inputs does the NTS (nucleus tractus solitaris) receive?
pharyngeal stimulation
gastric distention/irritation
what does the vomiting centre coordinate vomiting through?
interactions with cranial nerves 8 and 10
neural networks in NTS
what receptors are found in high concentrations in VC?
M1, H1, NK1, 5-HT3
what kind of nausea and vomiting is vestibular system responsible for?
motion and morning sickness
what kind of nausea and vomiting is CTZ responsible for?
chemotherapy induced
opioid induced
causes of nausea and vomiting?
infections
drugs
GI disease
pregnancy
increase in Ca2+
consequences of N+V?
dehydration
malnutrition
acid-base balance
aspirational pneumonia
oesophageal tear
what antiemetics are primarily used for treatment of pregnancy, PONV and motion sickness?
antihistamines
anticholinergics
what antihistamines are used for N+V?
H1 receptor antagonists
cyclizine, promethazine, diphenhydramine
how do antihistamines work to treat N+V?
block H1 receptors in vestibular nuclei
can exert blockade of muscarinic receptors
side effects of antihistamine use for N+V?
drowsiness - CNS depression/sedation
blurred vision
constipation, urinary retention
what anticholinergics are used for N+V?
muscarinic acetylcholine receptor antagonists
hyoscine hydrobromide
how do anticholinergics work to treat N+V?
block receptors at multiple sites
direct inhibition of GI movement and relaxation of GI tract contribute to antiemetic effect
