What is non-alcoholic steatohepatitis?
Fatty inflammatory changes to the liver, not associated with alcohol excess.
Outline the spectrum of NAFLD
Non-alcoholic fatty liver- fatty changes to the liver, fully reversible, not associated with inflammation
NASH - fatty changes associated with inflammation and fibrosis, not fully reversible + may lead to cirrhosis
What is the typical presentation of NASH?
Usually asymptomatic but MAY have hepatomegaly
What are 4 common risk factors for NASH/
- Obesity
- Diabetes mellitus
- HTN
- Hyperlipidaemia
How would you investigate NASH?
Bloods
o LFTs – generally hepatitic
o Lipid profile ↑
o BM ↑
USS abdo – snowstorm appearance
Biopsy – fat + inflammation
How would you manage NASH?
- Weight loss
- Exercise
- Statins
- Diabetes control
- 6/12 USS and AFB to check for HCC development
