Beta- agonists, sympathomimetics, anti-muscarinics, caffeine, theophylline, marijuana.

A
Hypovolemia, hyperthyroidism, sepsis, pyrexia, anaemia, acidemia, hypercarbia, hypoxia, pulmonary embolism, cardiac tamponade, alcohol withdrawal etc.

It is the camel hump like appearance of P waves in sinus tachycardia with very fast HR. For instance: 150 BPM
