Opioid antagonist, Antidote
Indications
Opiate overdose, complete or partial reversal of CNS and respiratory depression induced by opioids, decreased level of consciousness, coma of unknown origin
Naloxone Hydrochloride is indicated for the following drugs:
1.) morphine sulfate
2.) Heroin
3.) hydromorphone
4.) Methadone
5.) meperidine
6.) Paregoric
7.) fentanyl
8.) oxycodone
9.) codeine
10.) proxyphene
Narcotic agonist for the following?
Butorphanol, pentazcoine, nalbuphine
Adult dose / route
0.4 to 2 mg IM/IV/IO subcutaneous /ET/IN
Minimum single dose recommend: 2 mg
Ready at 5 minute intervals to a maximum dose of 10 mg
For in route, administer half the dose in each nostril.
Max dose is 1 ML per nostril.
Pedi dose / route
0.1 mg / KG per dose IV/IO/IM every 2 minutes as needed
Max of 2 mg if no response administer half the dose in each nostrils
Max 0.1 mg
Naloxone Hydrochloride incompatible with
Incompatible with bisulfite and alkaline solutions
Contraindications:
Use with caution in Narcotic dependent patients. Use with caution in neonates of narcotic addicted mothers.

Mechanism of action
Competitive inhibition at narcotic receptor sites. Reverses respiratory depression secondary to opiate drugs. Completely inhibits the effect of morphine.
Special considerations - True or false
Any use in breastfeeding women should be clearly indicated. Assist ventilations prior to administration to avoid sympathetic stimulation. Seizures without casual relationship have been reported. May not reverse hypotension. Use caution when administering to narcotic addicts. Half life of naloxone is often shorter than the half life of narcotics. Repeat dosing may be required. In cardiac arrest, naloxone is generally not beneficial

Naloxone hydrochloride Pedi / dose / route. 0.1 mg / kg per dose IV/kg per dose IV/IO/IM/ET every 2 minutes as needed. Maximum total dose of 2 mg. If no response in 10 minutes, administer an additional 0.1 mg / kg dose.
