Myopathy signs
Gradual onset symmetrical weakness
Generally proximal weakness initially
Preserved tendon reflexes
No parasthesia/bladder problems/fasiculations
Muscular dystrophies
Duchenne’s
Becker’s
Facioscapulohumeral
Myotonic dystrophy presentation
DM1 more common, DM2 rarer DM1 at 20-40yrs with distal weakness Myotonia (muscle spasms) Facial weakness + muscle wasting give long haggard appearance Cataracts
Myotonic dystrophy treatment + prognosis
Mexiletine may help with myotonia
Die in late middle age
Becker’s presentation
Similar to Duchenne’s but milder symptoms at later age with better prognosis
Facioscapulohumeral presentation
12-14yrs can’t puff out cheeks + raise arms above head
Ironed out expression
Up to 20% need wheelchair by 40yrs
Duchenne’s presentation
Presents at ~4yrs with clumsy walking, standing then respiratory failure
Pseudohypertrophy seen
