What is myocarditis?
Acute myocardial inflammation
Flabby heart with focal haemorrhages and inflammatory infiltrates
Causes of myocarditis
C infections - coxsackie, cytomegalovirus, Chagas disease, also adenovirus
Drugs causing hypersensitivity
Radiation
S/S of myocarditis
Palps, dyspnoea, HF, fatigue, chest pain
Investigations of myocarditis
CXR - cardiac enlargement
ECG - ST/T abnormalities, arrhythmias, heart block
Bloods - raised CK and troponin
Treatment of myocarditis
- Treat underlying cause - antibiotics where appropriate
- Bed rest when acute, avoid athletics for 6m
- ACEI, B-blockers, spironolactone, digoxin
Patient has a fever and pleuritic chest pain that is relieved by sitting up and leaning over
Pericarditis
Causes of pericarditis
viral, TB, uraemia, trauma, post MI/CABG (Dressler’s), connective tissue diseases
S/S of pericarditis
Fever, pleuritic pain, pericardial rub, pulsus paradoxus, flu-likw, dyspnoea, tachypnoea, tachycardia
Investigations of pericarditis
CXR
ECG - saddle-shaped ST elevation, PR depression
Treatment of pericarditis
NSAIDS - ibuprofen
