What is myeloma?
A
Cancer of the plasma cells (B-cells), monoclonal proliferation of abnormal Ig. Multiple myeloma affects multiple areas of the body.

What is monoclonal gammopathy of undetermined significance?
A
Excess of a single type of Ab without other features of myeloma.

What can monoclonal gammopathy of undetermined significance progress to?
A
Smouldering myeloma, premalignant.

What happens when myeloma invades bone marrow?
A
Suppression of development of blood cells - anaemia, neutropenia, thrombocytopenia.

What are the four key features of myeloma?
A
CRAB C - calcium elevated R - renal failure A - anaemia B - bone lesions/pain

How do you investigate suspected myeloma?
A
- FBC - low WCC
- Calcium - raised
- ESR - raised
- Plasma viscosity - raised
- Serum protein electrophoresis and urine Bence-Jones protein test
- MRI/CT/x-ray for skeletal survey for bone lesions

What is the management of myeloma?
A
- Chemotherapy - bortezomib, thalidomide, dexamethasone
- Stem cell transplant
- VTE prophylaxis
- Bisphosphonates and radiotherapy for bone disease
