Which disease will present with weakness of voluntary muscles that are commonly used, and worsened by increased use?
myasthenia gravis
What are some symptoms of myasthenia gravis?
diplopia, ptosis, difficulty swallowing.
What are some possible causes of myasthenia gravis?
thyrotoxicosis or thymoma in young women.
What are the screening tools for diagnosis of myasthenia gravis?
CT to rule out the thymoma. They can look for elevated acethylcholine receptor antibodies.
What are some treatments for myastehenia gravis?
anticholinesterase drugs- neostigmine
thymemectomy
corticosteroid treatments when the neostigmine does not work.
which drug should you not use in myasthenia gravis?
aminoglycosides
How does myasthenia graivs vary from lambert- eaton syndrome?
The weakness in lambert will get better with use
What are the circumstances that lambert eaton syndrom could occur?
could be a precursor to small cell cancer, or it can be an autoimmune disease
How would you confirm dx of lambert?
electrophysiologically because motor nerve increases remarkably if it is stimulated at high rates, even if it is weak.
How do you treat lambert?
plasmapharesis, immunosupressive therapy- prednisone
In duchenne type is dytrophin level normal of low?
low, due to the genetic defect on chromosome X for dystropin protein coding.
What is the difference btwn becker and duchenne?
becker will have normal dystropin levels.
What is the progressive inherited disorder that appears in the third or fourth decade of life commonly?
myotonic dystrophy
What would be the best confirmatory test of a myopathic disorder?
electromyography
Which muscles are commonly affected in myotonic dystrophy/
muscle stiffness showing delay before relaxing post contraction. They see wasing in facial, sternocleidomastoid, and distal limb muscles.
