Mx of COPD Flashcards
1
Q
What is the first line of medical management of COPD?
A
SABA or SAMA
2
Q
What is the second line of medical management of COPD?
A
IF
1. FEV1 > 50
- Regular LAMA /LABA
2. FEV1 < 50 - Regular LAMA/LABA
- ICS
3
Q
What is the final step of medical management of COPD?
A
1. Regular LAMA and 2. Regular LABA and 3. ICS 3/3
4
Q
When should long term oxygen therapy be considered?
A
Pa O2 < 7.3 kPa when stable
or
PaO2 < 8 with pulmonary hpt/peripheral oedema etc
5
Q
What is LTOT?
A
Continous O2 at home for atleast 15 hours a day
6
Q
What are the risks of administering O2 therapy?
A
Hypercapnia is cause of respiratory drive thus over ventilation can lead to loss of respiratory effort.
7
Q
What are the target O2 saturations in COPD?
A
- retain CO2 - 88-92% titrated by venturi mask
- not retaining CO2 and their bicarbonate is normal (meaning they do not normally retain CO2) then give oxygen to aim for oxygen saturations > 94%
8
Q
Which masks are used for oxygen therapy in COPD?
A
Venturi maks as they deliver a specific conc of oxygen.
