A

serial casting with long leg casts changed weekly for 4-6wks (ponsety method), percutaneous lengthening of achilles after casting complete, bracing for 2-4yrs after, surgery if needed.

A

pavlik harness (90degree flexion) before 6mo, monitor skin with cast or brace, encourage growth and development, 6-24mo need open reduction surgery and spica casting for tissue repair.

blood supply to femoral head is disrupted, causes necrosis and bone cell death and results in flattening of femoral head.

avascular necrosis for several months and might not know its happening. fragmentation when bone is disintegrating and tx at this stage, reossification when stronger bone develops, healing when bone growth complete and femoral head is reshaped.

strains is tendon and muscle, sprain is ligaments. Tx with RICE (rest, Ice, compression, elevation), nsaids, wrap limb distal to proximal, immobile for 10-14 days if severe.

reduce fracture either closed or open, immobilize, splints, cast, traction.

fracture in growth plate have high risk for deformity and poor healing, infection control to prevent osteomyelitis, can indicate abuse.

spiral- twist buckle/torus- inside of bone compresses from impact greenstick- bone bends and cracks on one side open- bone breaks through skin closed- bone doesnt break through skin comminuted- several fragments oblique- diagonal angle break through whole bone complete/transverse- 90degree angle complete break

compartment syndrome
fascia are tight, non stretching bands that divide the muscles. swelling causes pressure inside muscle group impairing tissue circulation and necrosis. casting can cause it from edema in cast

sx of compartment syndrome
pallor, pain, parasthesia, paralysis, pulselessness

tx of compartment syndrome
emergency. relieve pressure through cast. removal or fasciotomy.

overuse syndromes
microtrauma damage to bone, muscle or tendon from repetitive stress with no time to heal. examples are osgood schlatter disease, shin splints, severs, stress fractures. damage occurs in four stages

RICE and nsaids initially, stretching exercises, support devices, physical therapy, prevention, take time off between activities
