Biceps brachii
O: long head from supraglenoid tubercle (tubercle above glenoid cavity), short head from coracoid process of scapula
I: radial tuberosity and bicipital aponeurosis
A: flex and supinate forearm, flex arm at shoulder joint
N: musculocutaneous
Brachialis
O: distal anterior surface of humerus
I: ulnar tuberosity and coronoid process of ulna
A: flex forearm at elbow joint
N: musculocutaneous and radial nerves
brachioradialis
O: lateral border of distal end of humerus
I: superior to styloid process of radius
A: flex, supinate, and pronate forearm to neutral position
N: radial nerve
triceps brachii
O: long head from infraglenoid tubercle, lateral head from lateral and posterior surface of humerus, medial head from entire posterior surface of humerus inferior to groove of radial nerve
I: olecranon of ulna
A: extend foreram at elbow and arm at shoulder joints
N: radial nerve
anconeus
O: lateral epicondyle
I: olecranon and superior portion of shaft of ulna
A: extends forearm
N: radial nerve
pronator teres
O: medial epicondyle of humerus and coronoid process of ulna
I: midlateral surface of radius
A: pronates forearm at radioulnar joints and weakly flexes forearm at elbow
N: median nerve
pronator quadratus
O: distal portion of shaft of ulna
I: distal portion of shaft of radius
A: pronates forearm at radioulnar joints
N: median nerve
supinator
O: lateral epicondyle of humerus and ridge near radial notch of ulna (supinator crest)
I: lateral surface of proximal 1/3 of radius
A: supinates forearm
N: deep radial nerve
