Murmur in aortic stenosis?
Ejection systolic
Murmur in mitral stenosis?
Mid-diastolic
Murmur in aortic regurgitation?
Early diastolic
Murmur in mitral regurgitation?
Pan-systolic
Right-sided murmurs (tricuspid + pulmonary) are louder on
INspiration because blood goes IN to the right side of the heart
Left-sided murmurs (mitral + aortic) are louder on
EXpiration
because blood EXits the heart from the left side
What are the causes of a pan-systolic murmur?
Mitral/tricuspid regurgitation
VSD
What is the cause of a continuous machinery murmur?
PDA
What does S1 correspond to?
Mitral and tricuspid valve closure
What does S2 correspond to?
Aortic and pulmonary valve closure
What does a quiet S1 or S2 indicate?
Pericardial effusion/tamponade
What causes an S3 sound?
Blood coming into contact with a compliant (dilated) ventricle
What is S3 a sign of?
Can be normal in adults up to 30
- Mitral regurgitation
- HF
- Dilated cardiomyopathy
What causes an S4 sound?
Blood coming into contact with a non-compliant ventricle during atrial contraction (forceful contraction)
What is S4 a sign of?
- LVH
- Post MI fibrosis
- Failure of relaxation – diastolic heart failure
