What is heart failure?
Inability of the heart to adequately pump blood in relation to the body’s demands
What is systolic heart failure?
Impaired contraction
What is diastolic heart failure?
Impaired filling
Causes of systolic heart failure?
MI/ischaemia (commonest)
Dilated cardiomyopathy
HTN
Myocarditis
Causes of diastolic heart failure?
Pericardial effusion
Cardiac tamponade
Restrictive/hypertrophic cardiomyopathy
Left-sided symptoms of heart failure?
Fatigue, exertional dyspnoea, orthopnoea, PND
Right-sided symptoms of heart failure?
Fatigue, dyspnoea, peripheral oedema, ascites
Signs of left-sided heart failure?
Shifted apex beat, 3rd heart sound
Bibasal crackles
Signs of right-sided heart failure?
Raised JVP
Hepatomegaly
Ascites
Bilateral leg oedema
What investigations help diagnose heart failure?
Bloods - NT Pro-BNP
CXR
Echo
What is BNP?
Ventricular natriuretic peptide or brain natriuretic peptide is a hormone secreted by cardiomyocytes in the heart ventricles in response to stretching caused by increased ventricular blood volume
What is a raised BNP?
> 100
What is an echo used to investigate in heart failure?
Assess LV function
LV wall thickness
Ejection fraction
Valve disease
What is the ejection fraction?
How much blood the left ventricle pumps out with each contraction
CXR signs of heart failure?
ABCDE
Alveolar oedema Kerley B lines (interstitial oedema) Cardiomegaly Dilated prominent upper lobe vessels Pleural Effusion
