Mitral/Tricuspid Regurg
Mitral: Heard at the apex and radiates toward axilla
- MR: due to ischemia heart disease, dilation of the ventricle, and MVP
Tricuspid: Heard at the left sternal border at the tricuspid and radiates to right sternal border
- TR: due to dilation (IV drug user)
MR and TR most often caused by Rheumatic fever and infective endocarditis
Mitral Valve prolapse
systolic cresendo mumur with midsystolic click
click: sudden tensing of the chordae tympani
Heard over the apex
Caused by tissue degeneration (ehlers danlos, marfan), RF, chordae rupture
VSD
Systolic
Holosystolic blowing murmur heard at the tricuspid area
Aortic regurgitation
Diastolic Murmur
High pitched blowing murmur.
- Wide pulse pressure, head bobbing, bounding pulse
- caused by: dilation, bicuspid aortic valve, endocarditis, RF,–> progresses to heart failure
Mitral Stenosis
Diastolic murmur-follows an opening snap (due to abrupt stop in the opening of the leaflets) - rumbling murmur
LA»»LV during diastole
2ndary to RF, MS can result in LA dilation
PDA
systolic and diastolic
Machine like murmur in the infraclavicular area
Aortic Stenosis
Systolic Murmur
Cresendo-decresendo
Loudest at the base of the heart (Right sternal border) and radiates to the carotids
SYNCOPE, ANGINA, DYSPNEA ON EXERTION
* age related calcification or unicuspid valve or syphilliss
