What is multiple sclerosis?
An inflammatory demyelinating disorder of the CNS
Which form of MS does not have a relapsing and remitting course?
Primary progressive MS
What are signs associated with pyramidal dysfunction seen in MS?
- Dorsiflexion of the foot (Babinski sign)
- Muscle weakness
- Spasticity & Increased tone
- Hyperflexion
What are common clinical features of MS?
- Optic neuritis
- Sensory symptoms
- Urinary tract dysfunction
- Cognitive impairment
What happens when there is dorsal column loss in MS?
Changes in proprioception and vibration sensation
What is internuclear ophthalmoplegia?
Loss of function of the medial longitudinal fasiculus leads to failure of adduction of one eye and nystagmus in the abducting eye
What is present in the CSF in 90% of MS cases?
Oligoclonal bands (if they are absent be cautious of the diagnosis)
What are some ddx’s for MS?
- Vasculitis
- Glanulomatous disorders
- Structural lesions
- Infections
What is the first line disease modifying therapy?
Interferon Beta
- then Copaxone
What is the first line medication indicated in Relapsing Remitting MS?
