MSRA: Parathyroids Flashcards
What are the features of hypercalcaemia?
Renal stones
Abdominal pain
Polydipsia
Polyuria
Fatigue
Peptic ulceration
What are the features of hypocalcaemia?
Seizures
Muscle spasms
Perioral paraesthesia
What are the actions of PTH?
- Increased activation of vitamin D (takes place in kidneys)
- Bone breakdown
- Increased absorption by the kidneys (in exchange for phosphate excretion)
What is meant by pseudohypoparathyroidism?
Target cells insensitive to PTH
What are the blood results in secondary hypoparathyroidism from CKD?
Low calcium
Raised PTH (in response to low calcium)
Raised ALP (PTH stimulates bone breakdown)
Raised phosphate (kidneys unable to remove and excrete in to urine)
What is the first-line management in hypercalcaemia?
IV Fluids
What are the ECG changes in hypocalcaemia?
Prolonged QT interval
Name 5 causes of hypercalcaemia
Primary hyperparathyroidism
Malignancy
Sarcoidosis
Thiazide diuretics
Thyrotoxicosis
Dehydration
What is the management of severe hypocalcaemia?
IV calcium replacement (calcium gluconate)
