Movement Disorders Flashcards
Parkinson's
L-Dopa (levodopa)
Dopamine replacement therapy
Often combined with carbidopa + entacapone
Dental effect of L-Dopa and entacapone
Interaction with epi - tachycardia, arrhythmia, hypertension
Most common medication for Parkinson’s
Ropinirole
Ropinirole
Dopamine receptor agonist by mimicking dopamine
Selegiline
MAOB
Interactions with epi and narcotics (meperidine)
–> hypertension, tachycardia
Rasigiline
MAOB
Interaction with narcotics like meperidine –> hypertension and tachycardia
Amantadine
Enhanced release of dopamine
Usually with younger patients
Short effective period (~6 months) then switch
Classes of movement disorder repair
- replace dopamine
- dopamine receptor agonists
- block dopamine breakdown: MAOB
- enhance dopamine release
- muscarinic antagonists
Benztropine
Trihexyphenidyl
Anti-cholinergic aka muscarinic antagonists
3 dental tx considerations
- orthostatic hypotension
- early am appt ~90 minuses after L-DOPA
- cognitive decline
