Morphine Drug Classification
Opiate analgesic
Morphine Mechanism of Action
Opiate R agonist –> hyperpolarization of interneurons and decreased release of neurotransmitters responsible for the transmission of pain impulses to the thalamus and cerebral cortex
Morphine Onset of Action
Immediate IV, 15 - 30 minutes IM
Morphine Peak Effects
20 minutes IV, 30 - 60 minutes IM
Morphine Duration of Effect
2 - 7 hours
Morphine Half-Life
1 - 7 hours
Morphine Indications / Field Use
Severe pain
Morphine Contraindications
Hypotension, significant head trauma with AMS
Morphine Side Effects / Adverse Reactions
N+V, abdominal cramps, blurred vision, constricted pupils, AMS, headache
Morphine Dosing: (ADULT)
0.1 mg/kg slow (2 mg/min) IV/IO/IM, max single dose 10mg, q 10 min up to 3 total doses
Morphine Dosing: (ADULT > 65 years)
0.05 mg/kg slow (2 mg/min) IV/IO/IM, max single dose 10mg, q 10 min up to 3 total doses
Morphine Dosing: (PEDI)
0.1 mg/kg slow IV/IO/IM
Morphine Precautions
Morphine causes reduced chemoreceptor responsiveness to increases in CO2 – may lead to respiratory depression; caution in patients with hypersensitivity to analogs of morphine.
Morphine Drug-Drug Interactions
Additive effects (CNS depression) when given with antihistamines, antiemetics, sedatives, hypnotics, barbiturates, and alcohol.
