Major depressive episode criteria
*Depressed mood/anhedonia most of the time PLUS any 4 of the following over a two week period:
S: sleep changes (insomnia, hypersomia)
G: excessive guilt or worthlessness
E: lack of energy
C: Inability to concentrate
A: Appetite changes and/or weight changes
P: psychomotor agitation or retardation
S: Suicidal ideation
*Must cause significant impairment
*Not due to medical condition/medication/substance
Manic episode criteria
Distinct period of abnormally and persistently elevated or irritable mood AND persistently increased goal-directed activity or energy over at least 1 week period PLUS manifesting 3 of the following (4 if mood is only irritable)
D: distractibility
I: impulsive in risky behaviors
G: grandiosity
F: flight of ideas or racing thoughts
A: increased goal-directed activity
S: reduced need for sleep
T: talkative or pressured speech
*must cause significant impairement
*not due to medical condition or substance or medication
Mania vs hypomania
Hypomania follows the same DSM-5 criteria as mania except its over at least 4 days, doesn’t cause significant impairment, doesn’t require hospitalization, and doesn’t have psychotic features
Suicide risk factors
M: male
A: Age <19 or >45
D: depression
P: previous attempt
E: excessive alcohol or substance abuse
R: rational thinking loss
S: lack of social support
O: organized plan
N: non married
S: sickness
is manic episode considered an emergency?
yes due to loss of judgement and impulsivity that may cause harm to self or others
mood episode with mixed features? and Tx?
meets criteria for mania or hypomania PLUS at least 3 or the criteria for MDE with symptoms present nearly everyday over 1 week period.
responds well to anticonvulsant like Valproate acid instead of lithium
medical causes of MDE?
stroke/ MI
cancer
SLE
endocrine (hypo/hyptherthyroid, DM, hypo/hypercalcemia, cushing/addison)
parkinson
medical causes of manic episode?
hyperthyroid
temporal lobe epilepsy, MS
neoplasm
HIV infection
